This page documents the public web form opt-in process for InsurX Protect SMS messages so reviewers can verify the call to action, consent language, legal disclosures, and opt-in paths without guessing.

1. Public Opt-In Flow
- A user visits a public InsurX Protect web form, such as https://www.insurxprotect.com/forms/broad-s.
- The user enters their mobile phone number in the field labeled “Mobile Phone Number (optional)”.
- To opt in to SMS, the user must actively check an unchecked checkbox labeled “Optional SMS Consent”.
- The checkbox consent language includes the InsurX Protect brand name, message purpose, message frequency, message and data rates disclosure, STOP opt-out instructions, HELP instructions, support email, consent-not-required language, and links to the Privacy Policy and Terms & Conditions.
- If the user does not want text messages, they can leave the SMS consent checkbox unchecked and still submit the form, even if they provided a phone number for non-SMS advisor follow-up.

- Mobile phone numbers, SMS consent, and related opt-in data are not shared, sold, rented, or disclosed to third parties or affiliates for their own marketing or promotional purposes.
